Holy Names University Athletic Training
The Athletic Training Room is located in the rear of Tobin Gym next to the pool. We will open 2 hours prior to the start of competition unless otherwise requested.
The Athletic Training Room is equipped with a full-sized whirlpool (available upon request), combination ultrasound/electrical stim unit, ice machine, and hydrocollator. If your team will not be traveling with an athletic trainer, please send the necessary tape and supplies that they require with them.
A Certified Athletic Trainer will be present for all home competitions. Splints, crutches, and other emergency medical equipment will be available, if warranted. Shower facilities will be available for events on campus, although teams will need to supply their own towels. |

The forms below must be filled out by ALL student-athletes and returned to the Athletic Training Room by July 31st!!
NEW Student-Athletes: Print out and complete New Student Medical History. Please also review the Athletic Training Checklist
RETURNING Student-Athletes: Print out and complete Medical History Update.
ALL STUDENT-ATHLETES: Print out and complete Insurance Letter and General Info Form.
Completed forms can be dropped off in person to the Athletic Training Room, or mailed to:

Pre-Participation Physical Exams will be completed upon your arrival on campus before your team begins practice. Contact your coach for the date and time. |

Volleyball and Basketball
The Athletic Training Room will open 2 hours prior to competition, unless otherwise requested.
We will provide the following items during the competition: Water, Cups, Towels, Blood-kit/Emergency Equipment (Home bench)
If needed, emergency services will be provided through 911. |

Soccer
Home Soccer matches are played on the CSU, Hayward campus . The Athletic Training Room at HNU will open 3 hours prior to competition for treatments, unless otherwise requested. The Athletic Trainer will then be available on-field 1 hour prior to competition. Because our home field is off-campus, it will not be possible to provide any modalities within 1.5 hours of game-time.
We will provide the following items during the competition: Water, Cups, Towels, Emergency Equipment (Home bench)
If needed, emergency services will be provided through campus security/911. |

Cross-Country
If necessary, the Athletic Training Room can be opened for treatments prior to Home Cross-Country meets. If you would like your athlete to receive treatment the day of the event, PLEASE CONTACT THE ATHLETIC TRAINING ROOM at the number below. Because home meets are held off-campus, it will not be possible to provide any electrical modalities within 1.5 hours of the competition. A Certified Athletic Trainer will be present for the competition.
Please note: We are more than willing to provide taping services prior to Cross-Country meets. However, visiting athletes requesting this services MUST PROVIDE THEIR OWN SUPPLIES, as per the Cal-Pac Sports Medicine Guidelines.
